Referrals

The easiest way of organising a consultation with one of our surgeons is by submitting your self-referral online - just click on the button below to start. You will have an online questionnaire to fill in and will need to submit photographs of your hands.

After reviewing your photographs and questionnaire we will contact you to organise a consultation - in most cases this will be a telephone consultation, but you will be asked if you prefer a face to face consultation in the referral form.

Referral Login

If you have any difficulty submitting your photographs or filling in the online questionnaire, please contact us using our contact form. We will get back to you as soon as possible.

If you prefer to directly come to see one of our surgeons face to face at Spire Shawfair Park Hospital and do not wish to use our referral form, please use our contact form and our secretary will assist you to organise your face to face consultation.

  • Our online referral system is built to be secure and GDPR-compliant.

    You can print and fill by hand the questionnaires, and email them to us along with the photographs of your hands if you find it easier.

    We cannot guarantee that we will receive all the information - if you email us and do not hear back from us within 2 weeks please enquire using our CONTACT US form.

    We also cannot guarantee that information sent to us by email, including personal medical information, will not be lost or accessed illegally by third parties out of our control.

The surgeons of Edinburgh Dupuytren’s Group collect and store your personal, medical and billing data solely for the purpose of your care and associate administrative practices. Your data with EDG surgeons may be shared with other EDGroup members and staff or with other healthcare organisations including Spire Hospitals and Insurers when relevant solely for the purpose of your care and associated administrative business.

Please notice that while transmission of personal and medical information via our referral form is secure and GDPR compliant, we cannot guarantee that information exchanged via email is safely stored, not deleted, and not accessed by third parties without our permission or knowledge.
